Enrollment Procedure
(For New Enrollees)

Set a schedule for the entrance examination.
Wait for the confirmed schedule and take the exam on the assigned date.

Accomplish the Online Enrollment Form
Fill out the Online Enrollment Form or visit the Registrar’s Office.

Submit the following requirements:
■ Report Card (Form 138)
■ Birth Certificate (PSA)
■ Certificate of Good Moral Character
■ Four (4) pieces of 2×2 ID Photos
■ Baptismal Certificate (Catholic students)

Pay the registration fee
Payment may be made on-site or via authorized payment centers

Confirm enrollment
Confirm your enrollment with the Registrar’s Office.
